Swing Bed Program
Our Swing Bed Program (Skilled Nursing level of care) is designed for patients who are medically stable but still need short-term skilled therapy or nursing before returning home. Recover close to family and friends—right here in Fairview.
Why FRMC?
Our multidisciplinary team partners with patients and families to create a discharge plan that supports long-term success at home. Swing Bed can be a great option after acute hospitalization or inpatient rehab when you still need skilled services before a safe return home.

Who May Benefit
- After surgery (e.g., joint replacement, fracture repair)
- After a prolonged illness or hospitalization needing strengthening
- Wound care or IV therapy needs
- Neurologic or cardiac recovery requiring skilled therapies
How to Refer or Transfer
- Ask your current hospital case manager to contact FRMC Swing Bed for transfer.
- Providers may fax orders and clinical information (see fax below).
- Families can call us directly with program or insurance questions.
